The Hampshire and Isle of Wight End of Life Care Board includes people from local health and care teams.

They look after many different people, some who are at the end of their life.  They want to understand more about what really matters most to people at that time. Patients, families, carers, and groups that support them are all invited to join a community conversation to hear about what is important to you.

The theme for this conversation is: How can we improve how and when we identify people who are moving towards the end of their lives, to ensure that we can care and support
them and their families in the way they would wish.

Numbers for this event will be limited to enable all to have a voice and those registering will be advised once reviewed to ensure we enable representation from all areas.
