SGN Partnership helping to prevent vulnerable patient hospital readmissions

A pioneering project to prevent vulnerable patients from being readmitted to hospital in Hampshire and the Isle of Wight has been expanded, with help from SGN. One in five of all hospital admissions are made up of readmissions from vulnerable patients. The Home and Well partnership aims to address this by supporting patients with energy poverty needs and related social requirements back into safe and warm homes. COVID-19 vaccination for 5-11 year olds

During August, special child vaccination walk-ins are available at Hamble House at St. James's Hospital, Portsmouth. All children aged 5 to 11 can receive two paediatric (child) doses of the vaccine, with at least 12 weeks between doses. The paediatric vaccine is a different dose than that given to those aged 12 and over. It's important to remember that if any child has recently tested positive for COVID-19, they will need to wait 12 weeks before they can receive their vaccination. High levels of demand for urgent and emergency services at the Queen Alexandra Hospital site

Portsmouth Hospitals University NHS Trust (PHU) is currently experiencing high levels of demand for urgent and emergency services at the Queen Alexandra Hospital site. “Non-emergency attendances will not be seen in our Emergency Department and people will be redirected to the Urgent Treatment Centres at St Mary’s Hospital, Gosport war Memorial Hospital and Petersfield Community Hospital. Autumn Boosters for Care homes residents, staff and frontline care workers

The department of Health has issued a statement with an expectation that all residents and staff in Healthcare are eligible for the Autumn Covid-19 booster, and care homes need to start preparation to enable visiting NHS teams to maximise resident and care staff uptake when on site. The threat from flu still remains, and residents should get a seasonal flu vaccination, this will most likely be a co-administration of the flu and Covid-19 vaccinations when the NHS roving teams visit. Contacting the Care Quality Commission (CQC) if deaf or hard of hearing

The Care Quality Commission (CQC) is particularly keen to hear from deaf and hard of hearing people about their experiences and has teamed up with Disability Rights UK to help make this easier, they have various ways of contacting them by utilising forms, British Sign Language (BSL),  Sign Live and Text relay. Local people are being encouraged to share their thoughts on the standards of care they receive at GPs, dentists, hospitals and care homes, with the Care Quality Commission (CQC). NHS New Statutory Guidance for Working in Partnership with People and Communities

New Support Group for Men Survey

Before and during the Covid pandemic, the Good Mental Health Coop has been delivering courses, workshops and activities which focus on creativity and physical exercise to benefit our emotional resilience and overall mental health. We want to build on this and offer a new support group specifically for men, starting in September at the Buckland United Reformed Church.
